[BookMyShot] Services may ask you for personal and medical information such as your medical history, present condition or symptoms, and any treatments or medications that you have been prescribed. All personal and medical information that is collected is shared with your selected healthcare site in order to provide you with the selected healthcare service. Your personal and medical information will not be shared with any other party. However, anonymized data that does not contain any personal identifiers, may be shared with a third party for research or educational purposes, or utilized to improve our Services.

Accessing Your Personal Medical Information 

You have the right to access or amend any of your protected health information by contacting your selected healthcare provider. You can also contact our Privacy Officer in writing. BookMyShot may retain, erase, or anonymize personal and medical information as required by law or within a reasonable timeframe, not exceeding 6 months. You can continue to contact your selected healthcare site if you would like access to your personal and medical information after 6 months.

Withdrawing Consent

You reserve the right to change or withdraw consent to this privacy policy at any time. You can withdraw consent and have your records erased by contacting our Privacy Officer in writing. Unless you advise us otherwise, we assume that you consent to the collection, use, and disclosure of your personal and medical information as explained in this Privacy Policy.

How We Protect your Data

[BookMyShot] healthcare privacy is compliant with federal and state-level health information regulations, such as HIPAA (The Health Insurance Portability and Accountability Act), and Canadian healthcare regulations such as PIPEDA (Personal Information Protection and Electronic Documents Act). Our data service providers meet ISO 27001 independent security standards and have SOC2 & SOC3 security audits. In the process of delivering the Services, data may be processed outside of Canada, including the United States. If information is processed within the United States, we will ensure that all information will be processed in accordance with American healthcare regulations (HIPAA) and Canadian healthcare regulations (PIPEDA) to meet industry standard privacy and security laws. We also limit who has access to the personal and medical information that you provide to us. Your information is only shared with your selected healthcare site such as our pharmacy. No other healthcare sites will have access to the information and data. Each healthcare site also undergoes a verification check to ensure they are registered to provide the corresponding healthcare services.

Communications by Email

[BookMyShot] Services has a feature that sends a confirmation of a healthcare service to your email account. This may contain your name, the date of your healthcare service, the type of healthcare service received, and the healthcare site that provided the service to you. BookMyShot has no control over the security of your email. If you are unsure if your email will safeguard your personal information, do not use [BookMyShot] Services.

Children

We are committed to protecting the privacy of children. [BookMyShot] Services is not designed or intended to attract children under the age of 13. We do not collect personal information from any person we actually know is under the age of 13. A parent or guardian, however, may use the [BookMyShot] Services for a minor. The parent or guardian is solely responsible for providing supervision of the minor’s use of [BookMyShot] Services. The parent or guardian assumes full responsibility for ensuring that the registration information is kept secure and that the information submitted is accurate. The parent or guardian also assumes full responsibility for the interpretation and use of any information provided through the [BookMyShot] Services for the minor.
